Session Description

Healthcare professionals are increasingly recognizing that to provide patient-centered care, they must learn more about their patients to establish plans that align with known preferences related to future medical decision-making.


Advance care planning is the process of thinking about and sharing your values, goals, and wishes related to future healthcare decisions so that a person’s family and healthcare team can respect that individual’s personal choices if he/she is unable to communicate because of a medical condition. Advance care planning involves (1) reflecting on what matters most by discussing experiences, beliefs and choices; (2) creating a plan to communicate treatment preferences for future medical care, and (3) appointing a surrogate decision-maker. Attend this webinar to learn the value of advance care planning for patients/clients, healthcare providers, and for yourself. Discover the connection between advance care planning and advance directives. The presenter will also cover the importance of accurate and easily accessible documentation, storage, and retrieval of created advance directives so that an individual’s known wishes can be honored.

About the Speaker

Chris has been a masters-prepared healthcare social worker for almost 30 years, beginning her career as a perinatal social worker. In 1992, she started employment at Parkview Health, working as an inpatient rehabilitation social worker, hospice bereavement counselor, and then as the hospital’s first palliative care team social worker. In 2016, Chris began Indiana’s first hospital-based Advance Care Planning department. Chris serves as Respecting Choices® First Steps® ACP Faculty and as an Advanced Steps® Instructor. She just finished an appointed three-year term on the Indiana State Department of Health Palliative Care and Quality of Life Advisory Council.
